Shabbat Consent & Registration 

We are delighted to expand our on site service options!

Attendance at Shabbat services will be limited to VBS members only on a first come, first served basis and requires pre-registration and consent.

Only those members who are pre-registered for Shabbat will be permitted to enter the VBS campus. Everyone entering the VBS campus will be screened, and strict protocols of social distancing and face covering will be observed.  We will continue to stream services online for those who are unable to attend and want to stay connected with us.

To register to attend a Shabbat service at VBS, please complete the corresponding VBS member registration and consent form below. If you are having technical difficulties and need to register over the phone, please contact Yvette Ciaramilaro, Administrative Executive Assistant, at 818.530.4085. 

EVENING MINYAN
Evening minyan will be held on site Monday-Thursday at 6:00pm in Sher Lopaty beginning Monday, April 12, 2021. Minyan is open to the community and will be limited to 50 people on a first come, first served basis. No pre-registration required.

Pre-registration of VBS members is required to attend. Attendance is limited. Please select the date(s) you plan to attend on the form below.

Service will be streamed on Facebook and YouTube.

We are proud to announce that we are in the midst of the Jack and Rita Sinder Entrance Pavilion construction project that will not only beautify our front entrance but will make it more accessible to all of our members and guests. Due to this construction, there may be changes to the way you enter and leave the building. Access instructions will be made clear with corresponding signage directing you to the entrance. If you would like to make arrangements in advance to ensure your comfortable access to our services, please call Yvette during business hours at 818.530.4085.

Covid-19 Pandemic Consent and Registration
To Attend Services/Programs at Valley Beth Shalom (the “Consent”)

 
As with the transmission of any communicable disease like a cold or the flu, you may be exposed to COVID-19 at any time or in any place. Valley Beth Shalom (“VBS”) has established protocols for services and will be diligently following local, state, federal and CDC laws and guidelines for reopening – including disinfecting, social distancing and personal protection. You could become exposed to COVID-19 at any time or place despite VBS’ careful attention and policies.  In addition, despite VBS’ efforts, it is impossible to completely assure you that all other attendees will always follow these guidelines in perfect compliance at all times and it is required to complete and sign this Consent in order to attend services at VBS.  

I knowingly and willingly consent to attending worship services/programs during the COVID-19 Pandemic at VBS despite the risks outlined above and below.  

Initial below on behalf of the signer and the other household members:
 
I have read – and if applicable, shared with other members attending VBS services/programs – the “COVID-19 Protocols for Services/Programs” prepared by VBS (the “Protocols”).

I understand that the virus has a 14 day period during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to accurately determine who has the virus and who does not.

I understand – as do the other individuals listed -- that due to the frequency of visits to VBS of other members, staff, students and other visitors, and due to the characteristics of the virus, that I/we have a risk of contracting the virus simply from attending VBS worship services or other events.
I can confirm that as of the date of this Consent and as of the date that I visit the VBS campus, that neither I – nor the others listed on this Consent – am/are currently presenting (nor then presenting) any of the following symptoms of COVID-19 (the “Symptoms”) as listed below:

• Fever at or above 100.4 degrees Fahrenheit
• Shortness of breath
• Loss of a sense of taste or smell
• New or uncontrolled cough
• New onset of headache​​​​​​
-New onset of gastrointestinal issues
I confirm that as of the date of this Consent and as of the date that I visit the VBS Campus, that neither I -- nor the others listed on this Consent -- have tested positive for the Covid-19 virus, and that if I or the others listed on this Consent have tested positive for the virus within the last 14 days.

I confirm that I have not – nor have the others listed on this Consent -- been in close contact with anyone displaying these Symptoms in the past 14 days nor do I/we live with anyone displaying these Symptoms.
To prevent the spread of COVID-19 and to help protect others, I (and others listed on this Consent) agree to follow the Protocols upon entering and during my/our attendance on the VBS campus, including, but not limited to, the wearing of masks at all times, maintaining social distancing and following other rules of VBS that may change from time to time. 

I confirm that I have read and will follow the VBS travel and quarantine policy listed HERE.
By signing this Agreement, I acknowledge and consent to each statement above. In addition to signing this Agreement, I am providing all of the information below willingly so that, in the event VBS needs to contact me about any incidents regarding the virus at VBS, it will have this information available.  I am consenting on my behalf and on behalf of all of the members of my household that are listed on this Consent.
